Cannabis for Chronic Pain - What the Evidence Supports

Chronic pain is the most-cited reason for medical cannabis use across legal U.S. states - and one of the few cannabis indications with solid enough clinical evidence to be recognized by the National Academies of Sciences, Engineering, and Medicine (NASEM). The NASEM 2017 report concluded there is "conclusive or substantial evidence" that cannabis is effective for adult chronic pain. That said, "effective" is not "cure" - and the practical use of cannabis for pain requires careful product and dose selection. Cannabinoid + terpene selection for pain

For inflammatory pain (post-injury, joint, post-workout): caryophyllene-dominant + CBD-rich products. In preclinical and preliminary research, CBD has been studied for anti-inflammatory activity, and caryophyllene for CB2-receptor binding. These are research findings, not claims that any product reduces inflammation or relieves pain. Cannabis is not FDA-approved for pain - consult a licensed provider. See caryophyllene terpene guide.

For neuropathic pain (nerve pain, post-surgical): THC + CBD ratio products at 1:1 or 1:2 ratios. THC's CB1 effect modulates the central pain pathway.

For nighttime pain (interfering with sleep): myrcene + linalool + CBN profiles. Sedation pairs with mild analgesic effect. See CBN for sleep.

Practical recommendations

Start with CBD-dominant or 1:1 ratio products. If pain is poorly controlled, add a caryophyllene-rich live-resin or full-spectrum tincture. For joint/muscle pain, layer a CBD topical onto the affected area. If THC is needed for relief, microdose first (2.5–5 mg) before escalating. See microdosing cannabis.

Honest caveat

Cannabis is not a substitute for medical pain management. NASEM's "effective" rating is real but the effect size is moderate. Cannabis can interact with opioids, benzodiazepines, and other central nervous system drugs - meaning concurrent use should be discussed with the prescribing provider. Long-term high-dose THC use is associated with cannabis use disorder in 9-10% of regular adult consumers (CDC data) - meaning the dependency risk is real but lower than alcohol or opioid risk.
